Community Health Care Policy Survey 2026–2027

Daily Needs & Lived Experiences

To ensure our policy agenda is truly driven by the communities most impacted by health barriers, we at the Community Health Commission of Missouri (CHCM) are asking you to help us select our priorities for the 2026–2027 state legislative session. Your lived experiences will guide our actions.

This survey is estimated to take less than 5 minutes to complete.

At the end of the survey, you will be given the option of sharing your email to 1) receive a copy of survey results and/or 2) enter our raffle for a $50 Visa gift card.

Your information is confidential and your feedback is anonymous unless you choose to share your contact information at the end of this survey, for purposes outlined above. We use your responses only to inform our work, and results will only be shared in aggregate form.
