The Save and Continue survey toolbar adds an option for you to enter your email address to receive a link to return to your survey later.
The toolbar will be added to all pages after the first page of your survey.
The eligibility requirements for this application include:
- Has had staff or a team of staff trained on school-wide positive behavioral interventions and supports (SW-PBIS)
- Has 3 years of trend data on fidelity and have demonstrated fidelity of implementation of SW-PBIS as measured by the Tiered Fidelity Inventory (TFI) or Benchmarks of Quality (BoQ) with the most recent measurement within the past year.
- Overall TFI score of 70 percent or above
- Overall BoQ score of 70 percent or above

Please submit your application April 2024. Individual schools who meet Sustaining Exemplar status will be notified in May of 2024.
