Core Services Reporting Requirements:
i. Subrecipient will develop a proposed Annual Work Plan. The Work Plan will identify goals that will be completed by the Subrecipient during the period of the grant. Each goal will identify tasks required to accomplish the goal with timelines and/or frequency of tasks to be undertaken. The subrecipient will submit a Work Plan via *online webtool “Alchemer”.
Recovery Coach Services Reporting Requirements:
- Recovery Coach program activities, goals and timelines will be included in the Alchemer Work Plan webtool. The Work Plan is due 30 calendar days following grant execution, using the online Alchemer reporting tool.
