Your answers to these questions will help your teachers and school learn how they can better support students in school. The questions ask about, for example, your views on how you learn in class, how hard or easy it is for you to think about and solve problems, and your thoughts on your classroom experience.
It is up to you whether you want to answer the questions. If you choose NOT to give answers, it will not affect your grade in your class. If you do not feel comfortable answering one question or are unsure how to answer a question, please do not answer and go to the next question.
Your teacher or principal will NOT see your answers; your answers will be added to the answers of all the students in your grade. Your school will use all student answers to better understand your classroom experiences.
There are no right or wrong answers. The survey will take 15 to 20 minutes to complete. Please ask for help if you are not sure where or how to mark your answers to these questions. You may also ask for help if you are unable to read a question.
Please think of your English, Math, Science, and History lessons when you answer these questions. Please answer honestly so your teachers and school will know how best to support you.
