Opioid Antagonists (Narcan) for Community Programs Screening

Thank you for your interest in receiving opioid antagonist medication for your organization. Currently the Department of Health offers Narcan, a lifesaving medication which can be given by anyone to temporarily reverse an opioid overdose. In an effort to streamline the process of becoming a new site with our program, we have created this form to obtain the initial information needed for our team to begin the process.
