I am participating as a volunteer in Back to School PBC! at my own risk, and in consideration for myself participating in Back to School PBC!, presented by Palm Beach County, through its Office of Community Revitalization, the Spirit of Giving Network, the Children's Services Council of Palm Beach County, the Palm Beach County Sheriff's Office, Palm Beach State College - Belle Glade Campus, and the Edna W. Runner Tutorial Center (hereinafter the “Collaborative”), I agree to be solely responsible for any injury, loss or damage that I sustain as a result of my voluntary participation in the event, even though such injury, loss or damage may be caused by the negligence of others. I also agree to protect, defend, reimburse, indemnify and hold Palm Beach County and the Collaborative, its agents, designees, employees, and elected officials harmless at all times from and against all claims, liability, expenses, losses, costs, fines, damages or causes of action of every kind and character, including attorney's fees and costs, whether at trial or appellate levels or otherwise, arising during, as a result of, or in connection with my participation in the Back to School PBC! event. By attending this event you also hereby give consent to Palm Beach County for the purpose of being interviewed and/or photographed for print publications, website, radio or television broadcasts. You hereby release, discharge and agree to hold harmless Palm Beach County for any liability by virtue of my consent to appear in these photographs, videotapes or interviews. I certify that I have read and fully understand this release, and I am of lawful age and legally competent to enter into this agreement.

I am the parent or legal guardian of a volunteer under 18-years of age (the “Minor”) who is participating as a volunteer in Back to School PBC!. In consideration for the Minor’s participation in Back to School PBC! presented by Palm Beach County, through its Office of Community Revitalization, the Spirit of Giving Network, the Children's Services Council of Palm Beach County, the Palm Beach County Sheriff's Office, Palm Beach State College - Belle Glade Campus, and the Edna W.
