2021 Exemplar PBIS Application Cohort

Thank you for applying to become a PBIS Sustaining Exemplar School. 
This application is for individual school recognition for those that were cohort trained by the state.

The eligibility requirements for this application include:

• Completed two-year SW-PBIS training through a cohort with the Minnesota Department of Education or one of the Regional Implementation Projects
• Demonstrate fidelity of implementation as measured by the Tiered Fidelity Inventory (TFI), the Benchmarks of Quality (BoQ) or School-wide Evaluation Tool (SET)
  • Cohorts 1-13 must have taken a fidelity assessment within the last 12 months.
  • Cohort 14 in order to be eligible must have taken one outside of training (7/1/20-present)
    • Overall TFI scale score of 70 points or above
    • Overall BoQ scale score of 70 points or above
    • Overall SET scale score of 80 points or above

Please submit your application by April 9, 2021. Individual schools who meet Sustaining Exemplar status will be notified at the beginning of May 2021.
